Donor is allergic to the following:

Yellow Hornet, Yellow Jacket, Honey bee, Paper wasp

Alternaria alternate, Common ragweed, Kiwi, Papaya, Mango

Prior testing has detected significant IgE antibodies to the above allergens. This lot of plasma has not been tested.
